"Insurance certificates can be accessed on the provider's (Arthur J. Gallagher & Co.) online portal. Subscribers can access this on the Benefits page of ustacoaching.com and clicking on the first tile "USTA Coaching Exclusive Benefits". Click on the 'Insurance Certificate' link, which will direct you to a landing page where
your Certificate can be downloaded. We recommend saving a copy to your personal
drive or folder for easy access in the future."
How do I request an Additional Insured (AI) COI?
- Send all requests via email to Gallagher Affinity – Sarasota.bsd.operations@ajg.com . Please include all relevant details, such as the name and address of the requestor, as well as the start and end event if the request is for a specific event.
How long does my general liability insurance policy run?
- Your policy will be valid for one full year and will align with your USTA Coaching Subscriber dates.
How long does it take for insurance benefits to activate?
- Insurance benefits through Gallagher will take up to 24 hours to activate after your subscription purchase, and after Safe Play approval. You'll receive confirmation when your benefits are active, accessible through ustacoachingbenefits.com using Single Sign-On (SSO). Please note that insurance benefits are not available for international customers.
What are the requirements to be covered under the program?
- To be covered, you must be an active USTA Coaching subscriber in good standing. If you choose not to renew or to cancel your USTA Coaching subscription, your general liability coverage will cease immediately. No premium refund will be provided.

Which specific activities or racquet sports are covered under the General Liability Insurance Policy?
- The General Liability Program applies to registrants on file with the Policyholder (USTA) for your tennis, racquet, and paddle sport (pickleball, padel, platform tennis), including coaching, teaching, playing, and officiating activities.
Does the insurance include event coverage for coaches who run summer camps, clinics, tournaments, etc.?
- Yes, the general liability insurance covers events organized by the covered coach. Coaches can request policy details from AJ Gallagher directly at (866) 978-2971. Their hours of operation are Monday - Friday 8am - 5pm CST.

How much is the Accident and Disability coverage?
- Participant Accident Insurance
- Accidental Death and Dismemberment (AD&D) - $25,000
- Accident Medical Expense - $50,000 per person per accident ($500 Deductible)
- Weekly Accident Indemnity - $500 per week up to 52 weeks (max $26,000)

USTA Coaching offers health insurance. How do I access it?
- USTA Coaching will offer access to a health insurance marketplace, which will help coaches aggregate options for health insurance based on their state and individual health insurance needs. The cost of health insurance is the responsibility of the coach. More information can be found on the MY Benefits page. To access the my benefits page login to ustacoaching.com an click on your account icon in the upper right corner. Click Manage on any benefit labeled with insurance and then click on Insurance Marketplace.

How do I file a General Liability insurance claim?
- A downloadable claim form is available via your member portal. Click on the “Claim Form” link. You will have to add your contact information, provide a location address of where the claim took place and include a general description of the occurrence. Once completed, save the document and send it to gps.lwr.usta.service@ajg.com.
